A Provider with over 200 primary care and specialty physician practices, children’s specialty clinics, home health care, behavioral health, occupational health, rehabilitation services and urgent care facilities wanted to improve care management, outreach and back office processes for Gastro-Intestinal patients.
Solution
Analyzed paper-based in-take, assessment, care plan and follow-up converting to Salesforce HealthCloud forms using MuleSoft for data exchange with Cerner and Epic EHR platforms. Patient profiles integrated into Salesforce Marketing Cloud to manage alerts, notifications, communications and interactions.
